Please use this form to help us track your volunteer service hours. If you volunteered multiple times in one month, just indicate the TOTAL number of hours for the month. You may fill this form out as many time as you want per month, i.e. if you prefer to fill out it out daily or after each time you volunteer, you do not need to wait until the end of the month.
FOR HOURS VOLUNTEERED,THE CATEGORIES ARE DEFINED AS FOLLOWS:
These are hours you volunteered acting in the capacity of your paid profession. For example, if you are an architect, you provided volunteer architectural design services. If you are an architect and you performed grant research or data entry, those hours are NOT professional hours.
EVENTS OR OTHER NON-PROFESSIONAL:
These are hours you volunteered assisting with events (e.g. outreach/awareness events, conferences, fundraisers, etc.) and/or hours where you provided a service outside of your professional skills. For example, hours that a volunteer who is a nurse (by profession) spends talking to event sponsors and writing thank you letters are NOT professional hours.
If you have “Professional” and “Events or other Non-Professional” hours to log, you must fill the form out twice – once to log the “Professional” hours and again to log the “Events or other Non-Professional” hours.
